Replacement Card Policy:

Safety Training Plus understands that sometimes people lose their card or your card becomes lost, or gets stolen. We have a solution. You can now have an "original" replacement card sent to you by filling in the form below. You can order your "replacement" card 24 hours a day, 7 days a week.

Use our replacement form below, and get your card replaced soon.

Please allow up to 2 weeks for card processing. Card processing will start once we receive your fee. Requests made after normal business hours, will be handled the next business day. Office hours are M-F 9am-4pm PST.

To receive your replacement card, simply submit the form below and click on the Pay Now button or mail a $20.00 check or cash to:
